CHIS Questionnaires

California Health Interview Survey (CHIS) questionnaires are available at the public. Below have full questionnaires of web-based additionally telephone surveys for adults, adolescents, and children startup for 2019. In addition to English, CHIS is conducted in Spanish, Chinese (Mandarin and Cantonese dialects), Korean, Vietnamese, and Tagalog.

Separate interviews are conducted for adults (age 18 and older), adolescents (ages 12–17) and children (birth through 11 years of age). For children, CHIS interviews the parent or guardian in the domestic who exists most knowledgeable about the child's health.
